This report offers practical guidance that incorporates feedback from the field to providers and payers to help them get started on the integration of social determinants as part of their core business. Through case study analysis, the authors find that initial social service investments have the potential to yield high financial returns within a time frame of 18 months or shorter. The authors suggest that the next frontier in the social service investment space will be to prevent individuals from becoming high-need, high-cost in the first place, rather than seeking them out once they have already passed that threshold.
