Description
This report is a case study of a transitional respite care program that uses recouperative care beds provided by homeless shelter for local hospital to which to discharge homeless post-op patients. The Transitional Respite Care Program (Respite Program) in Spokane, Washington, is a collaboration among Catholic Charities Spokane, Providence Sacred Heart Medical Center (part of the Providence Health & Services health system), MultiCare Deaconess Hospital, and Volunteers of America. Since 2013, this clinical-community partnership has provided post-hospitalization care and tailored, coordinated services to 678 homeless clients, giving them an alternative to the ED and a better chance at healing.
